An exciting and rewarding opportunity has arisen to join Bristol Adult Social Care as a Social Care Practitioner.
We have permanant vacancies available in First South on both a full-time and part-time basis.
We work with citizens living in the south of the city to carry out initial assessments where people are new to adult care or have no existing commissioned service in place.
The team works with people in a time limited way so the work is fast paced and provides excellent opportunities for variety meaning no two days will be the same and there will always be an opportunity for challenge and development. We manage a range of cases with varying complexity.
You will have the opportunity to be creative in your work, working with citizens to effectively problem solve and maintain control over their lives and support.
You will be working within the statutory structure of the Care Act 2014 undertaking, Care Act Assessments/Carers Assessments/Mental Capacity Assessments and Best Interest Assessments. The team are also responsible for carrying out s42 Safeguarding Enquires.
You will work closely with other agencies and departments across adult care.
You will work within a team of team manager, senior practitioners, social workers and social care practitioners and will receive guidance and support from your colleagues to further develop your skills, knowledge and confidence in this fulfilling role alongside professional supervision.

MI Citywide is a specialist multi disciplinary adult care reviewing team consisting of occupational therapists, social workers and social care practitioners.
We work with citizens who are in receipt of commissioned services and direct payments across Bristol and out of area carrying out planned assessments and reviews under the Care Act 2014. The team are also responsible for carrying out S42 safeguarding enquires. We carry out mental capacity act related work.
We work in a time limited way and our work is fast paced and varied which can be complex and challenging. We support Bristol citizens to achieve positive outcomes and maximise choice and control in their lives.
There will be opportunity to work closely with key partners both internally and externally. Our team focus is to maximise the independence of Bristol citizens and carers through use of social capital, Tier 1 and 2 services and Tec enabled solutions.
You will receive guidance and support to develop your knowledge and skills through regular professional supervision alongside support from your team colleagues as well as opportunities for training.
