Due to the promotion of one of our Referral Coordinators, an exciting opportunity has arisen for a Permanent Referral Coordinator role based at South Intermediate Care Centre covering a City-Wide service.
We are looking for someone to work alongside the other 3 referral coordinators in providing a high quality first point of contact for prospective referrers.
Screening referrals coming into the Reablement Service in a timely manner for appropriateness and inputting all relevant information onto ICT systems.
Reablement service helps people to retain or regain their skills and confidence so they can learn to manage again after a period of illness.
The service is usually provided in the person’s own home by a team of social care professionals. Reablement is an approach that, irrespective of diagnosis, aims to assist people to continue to live as they wish. It seeks to enable the individual to do ordinary activities like cooking meals, washing, dressing and moving about the home.
Reablement is used to support discharge from hospital, prevent readmission or enable an individual to remain living at home.

For employees who are currently on a fixed term contract with Bristol City Council, the secondment must not extend beyond the end of the fixed term period to ensure there is a substantive post to return to.
If you are currently a casual worker or a member of an employment agency (currently working with Bristol City Council) this post will be a fixed term contract.
